NOTICE OF CHARGES and HEARING ON THE MERITS

Dear Mr. Sloan:
Please be advised that the Taxi and Limousine Commission ("TLC") will be seeking the discretionary revocation of your TLC license (# 5093363) pursuant to Section 8-14 of Adjudications Procedures of the Taxi and Limousine Commission.
A hearing on the merits will be held, which may result in substantial fines, suspension and/or revocation of your license.
The hearing will be based upon the following charges.
1) You are charged with violating For-Hire Vehicles Rule 6-18(d)(1) in that on September 7, 2 00 1, you committed an act of misrepresentation by falsely stating on Question 18 of your application for a TLC license (# 5093363) that any license issued to you was never suspended or revoked.
2) You are further charged with violating For-Hire Vehicles Rule 6-18(d)(1) in that on September 7, 200 1, you committed an act of misrepresentation by falsely stating on Question 20 of your application for a TLC license 5093363) that you were never convicted of a crime.
The above charges are returnable on Friday, July 12, 2001 at 11:00 a.m. at the TLC Tribunal located at 40 Rector Street 8th Floor, New York, New York, where a hearing will be conducted. You may be represented at this hearing by an attorney or an authorized representative of your choice. You may present evidence in support of your defense and/or witnesses on your behalf. Failure to appear will result in the hearing being held in your absence and a default judgment.
